Install a Monument

A permit to install a monument (PDF) must be completed before any gravestone installation or foundation work is performed.  To obtain Cemetery Committee authorization, the installer may return the completed form to the Town Office via fax, post, in person, or email.  Signed permits will be kept on file at the Town Office.  

Lot Ownership Transfer

Complete the Cemetery Lot Ownership Transfer Form (PDF) to transfer ownership of a cemetery lot.

Open a Gravesite

This permit to open a gravesite (PDF) must be completed and delivered to the Damariscotta Town Office or to a member of the Cemetery Committee, and permission must be obtained, prior to the opening of any grave at Bethlehem, Hillside, or Walpole Cemetery.  There is no fee associated with this permit.  Please note that this permit is separate from the state’s burial permit, Disposition of Human Remains, which may be obtained from the Town Clerk upon payment of the applicable state fee.
